SFAN Outreach Worker

The South Florida AIDS Network/Outreach workers must work with service providers, including medical case managers, to locate people lost to medical care or medical case management and bring them back to care. The medical case manager, or pharmacy staff, after repeated attempts to contact the client by phone and mail without success, may refer the case through a Service Delivery Information System (SDIS) certified referral to an outreach worker.
High School diploma and/or 1-3 years experience required; Bachelors degree preferred. All staff providing Peer services must be certified through the State of Florida's Department of Health HIV/AIDS 104 course. HIV field experience preferred. The person must be able to work late evenings and weekends. Preference will be given to those who are bilingual (English / Spanish or Creole). Must have his/her own transportation. The person will work in the community at large and in specific areas of Miami-Dade County. American Heart Association (AHA) Basic Life Support (BLS) for Healthcare Providers certification with at least 6 months validity required.
