Selection Criteria:
- Bachelor degree from an accredited university in a health or human service area of study.
- Current Case Management Certification (CCM) and/or Social work licensure are a plus.
- Bilingual English/Spanish or English/Creole fluency.
- Able to speak, read and write in both languages without limitations or assistance.
- Knowledge of local community health and social service agencies, programs and resources, and experience arranging community resources for patients.
- Possess intermediate to advanced computer skills and experience with Microsoft Word, Excel and Outlook.
- Knowledge of state and federal statutes, rules and policies that affect the targeted populations.
- Use of private vehicle for company business.
- Valid state driver's license and proof of personal vehicle liability insurance with limits set by the organization.
- Required: Minimum of two (2) to five (5) years of relevant experience within a community health setting managing populations with complex medical and/or behavioral health needs.
- Prior experience with electronic documentation, and experience documenting in multiple computer applications.
- Prior experience and working knowledge of Medicare, Medicaid, managed care and commercial payor principles, and benefit coverage and limitations.
- Prior experience working with uninsured populations.
- Desired: Prior experience documenting in an electronic medical record (EMR).
- Prior Case Management experience.
- Completion of AHCA approved mental health targeted case management training.
